Summary You will function as the Contract Nursing Home Registered Nurse for the Amarillo VA Health Care System (AMVAHCS. Responsibilities You will communicate with facility staff and assist to facilitate timely services for Veterans in their care. You will provide active participation in the consultation process for specialty services that may be required for the care of Veterans that have been placed in contracted long-term care facilities. Knowledge of long-term care regulatory requirements in the community. You will perform as a liaison between AMVAHCS and all contract nursing homes in the AMVAHCS catchment area. You will make on site visits to all contract nursing homes caring for placed Veterans to assure standards of care are being provided. You will act as a point of contact for receiving consults/assessments for Veterans placed in contract homes and SVH for additional DME that may be required. You will be an active participant in the screening committee for veteran placement in contract long-term care facilities. You will be a participating member of the survey team for the State Veterans Home and VA liaison with on-site SVH representative. Maintain COR level II training (Contracting Officer Representative). Attend meetings for VISN and VACO for SVH and CNH and report information to the GEC Chief. Deactivate or reactivate by notifying HAS, pharmacy and clinic team to change access to primary and pharmacy at the VA. Report information and changes to the Chief of GEC on issues with SVH, CNH changes in Star ratings, state reportables, and issues related to consults for specialty services. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: -Geriatric experience 2 years is preferred -Providing education and counseling to patients and families -Multi-disciplinary experience and collaboration -Document follow up care of all contracted Veterans to community nursing homes -As liaison build a rapport with facility staff in the contract homes requesting additional information when necessary and placing consults as requested. -Computer literate with typing skills, experience with word, excel, and Outlook -Positive participation as a team member Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Physical Requirements: The position requires moderate lifting and carrying (15-44 pounds), straight pulling (3 hours), pushing (3 hours), reaching above shoulder, use of fingers, both hands required, walking (8 hours), standing (3.5 hours), kneeling (0.5 hours), repeated bending (1 hour), depth perception, ability to distinguish basic colors, and hearing (aid permitted). Education The Associate Degree Nursing (ADN), Bachelor of Science degree in Nursing (BSN), or Master of Science degree in Nursing (MSN) must be fully accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission On Collegiate Nursing Education (CCNE).
