Job Description Summary

This is a 100% full-time role that serves in a family nurse practitioner practice position in the College of Nursing to provide direct care in an innovative partnership with at Bluebonnet Trails Community Services, a local mental health authority in Central Texas. The family nurse practitioner is responsible for the delivery of professional and competent care to individuals in need of substance use disorder treatment including medication assisted treatment (MAT) and related primary care.

This position is funded from grant and/or contract funding which is renewed under the provisions of the grantor of the contract. The current grant and/or contract funding is through 2/14/2022. This position, therefore, is contingent upon continuation of funding of these grants and/or contracts and satisfactory job performance.

Required Education:

  • Master of Science degree in nursing or higher with national board certification as a family nurse practitioner or equivalent combination of education and experience.

Required Experience:

  • One year directly related experience serving as a family nurse practitioner

Required Licenses and Certifications:

  • Requires an active license as a Registered Nurse and as an Advance Practice Registered Nurse, Family Nurse Practitioner, in the state of Texas with prescriptive authority and national board certification as a family nurse practitioner with a DATA 2000 Waiver to prescribe medication assisted treatment (MAT).

Required Special Knowledge, Skills, and Abilities:

  • Ability to multi-task and work cooperatively with others

Preferred Experience:

  • At least five years of clinical nursing experience is required.

  • American Heart Association - Basic Life Support Certificate.

  • Experience with computer systems required, including web based applications and electronic health records, some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access.

  • Experience in health professions education is preferred.

  • Experience working with diverse individuals and communities

Job Responsibilities

  • Performs history, psychosocial, and physical examinations, diagnosis acute and chronic primary care conditions

  • Formulates therapeutic plans of care, ordering and interpreting laboratory and diagnostic tests, primary care medication management and provides education to patients and their families.

  • Coordinates the care of patients in order to provide safe, effective, efficient and timely patient-centered care.

  • Liaises between clinical staff and consult services.

  • Care plans and execution of those plans should encompass the entire service to tie the service together.

  • Consults with physicians and other members of the healthcare team regarding the care of patients with complex diagnosis.

  • Serves as a clinical resource for staff through consultation formal and informal education, review of literature and participation in seminars, conferences and simulation.

  • Collaborates with both family practice and psychiatry physicians

  • Document clinical encounters clearly, precisely and in a timely manner.

  • Works in a collaborative relationship with other health professionals to determine health needs of patients and families.

  • Demonstrates and role models an interdisciplinary collaborate approach to patient care.

  • Coordinate the patient care plan and communicate with the integrated behavioral healthcare team.

  • Applies, models and teaches evidence-based Screening, Brief Intervention, and Referral for Treatment SBIRT in clinical setting.

  • Promotes costs savings and optimal patient experience in primary care

  • Participates in rapid cycle continuous process improvement

  • Participates in grant data reporting

  • Participates in scholarly projects as appropriate

  • Preforms other duties as assigned.
