This position is located at our BonitaView Hospice Home in Bonita, San Diego County.

Hours

12 hour shifts, 7:00am - 7:30pm, working weekends as needed.

Required Skills and Qualifications

• High School Diploma

• Current unencumbered State of California Licensed Vocational Nurse licensure

• Current BLS certification for Healthcare Providers issued by the AHA

• Minimum of one year LVN experience

• Fluent knowledge of English, both verbal and written.

• Knowledge of nursing process.

• Effective interpersonal and communication skills

• Ability to perform all baseline clinical nursing interventions

• Fluent knowledge of English, both verbal and written.

• Current registration with State of California Board of Vocational Nurse.

Preferred Skills and Qualifications

• Acute care hospital LVN experience

• Hospice and/or Home Health experience

• Strong computer skills and efficient use of EMR

• Strong communication, team work, problem solving and critical thinking

• Efficient time management skills, flexibility

• Ability to work in a completely autonomous environment

• Bilingual English/Spanish

Summary

Under the direction of the registered nurse, the LVN assists with performing and overseeing patient care through the use of the nursing process. The LVN will provide nursing and supportive care to patients and their families during end of life in our 6-bed, general inpatient hospice homes. Within scope of practice and under the supervision of RN. The LVN performs assessment, all baseline clinical nursing interventions, actively participates in the patient care plan, evaluation of pain and symptom management and provides care at end of life. The LVN collaborates with the charge RN and hospice IDT to ensure effective and safe patient care is implemented. The LVN works in highly autonomous environment.

Sharp Hospice Care is dedicated to providing comprehensive care and compassionate support to individuals facing life-limiting illnesses, along with their families and friends. Services range from relief of physical discomfort to emotional and spiritual support. Sharp Hospice Care also provides home based palliative care consultation for patients with advanced illness such as heart failure, chronic obstructive pulmonary disease and dementia. The homes – staffed 24 hours a day by licensed nurses – provide a comfortable and supportive environment for our patients and their families.
