Humana Care Manager, Telephonic Nurse 2 (Work at Home LA or MS) in Metairie, Louisiana


The Care Manager, Telephonic Nurse 2 , in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Manager, Telephonic Nurse 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.

The Care Manager Telephonic Nurse 2 employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. This role involves targeted telephone outreach calls to members that support quality stars measures and completion of recommended annual health screenings.

  • Focus on closing gaps in care by identifying and resolving barriers that hinder effective care

  • Address social determinants of health

  • Act as an advocate on the member's behalf by providing resources and patient education to help maintain a healthy lifestyle

  • Foster an environment that can provide the necessary and quality healthcare for our members to allow them to live out their best life

Required Qualifications

  • Licensed Registered Nurse (RN) or MSW in the (appropriate state) with no disciplinary action

  • 3-5 years of clinical experience preferably in an acute care, skilled, rehabilitation or home health clinical setting

  • Must be passionate about contributing to an organization focused on continuously improving consumer experiences

  • Ability to work independently under general instructions and with a team

  • Ability to attend scheduled trainings and team meeting on location as needed

  • Ability to access and incorporate information from multiple systems

  • Comprehensive knowledge of Microsoft Word, Outlook and Excel

  • Excellent communication skills both verbal and written

  • Must have the ability to provide a high-speed DSL or cable modem for a home office (Satellite and Wireless Internet service is NOT allowed for this role). A minimum standard speed for optimal performance of 10x1 (10mbs download x 1mbs upload) is required

Preferred Qualifications

  • Education: BSN or Bachelor's degree in a related field

  • Previous Health Plan experience

  • Experience with case management, discharge planning and patient education for adult acute care

  • General knowledge of HEDIS and other Quality Stars measures

  • Certified Case Manager (CCM) a plus

  • Hours for this role are: Monday-Friday 8am-5pm CST
