JOB REQUIREMENTS: Position Description: Position Identification Advanced Practice Provider Job ID#: 28358 Position Category: Patient Care Position Purpose Advanced Practice Nurse Provides specialty health care services within defined scope of practice in collaboration with a licensed physician and in accordance with MCW/Froedtert Health policies and procedures. Department Position Purpose The Advanced Practice Provider (APP) works as part of a multidisciplinary health care team, providing a broad range of diagnostic and therapeutic services for the patients of the outpatient services as a member of the Department of Medicine. The APP will have a strong foundation in evidence based practice and ensure an environment of safe quality care. The APP in this position will work in collaboration with palliative care physicians and other palliative care team members, to deliver palliative care, hospice care, and related health care services to patients and families. They will assist in the coordination of care for patients seen by the Palliative Care and Hospice Care Consultation Service primarily in the outpatient setting. Physician Assistant Provides specialty medical care within defined scope of practice under the supervision of a licensed physician and in accordance with MCW/Froedtert Health policies and procedures. Organizational Structure Location: Froedtert and Medical College of Wisconsin Clinical Cancer Center Department: Medicine Division: Geriatric and Palliative Medicine Supervising / Collaborating Physician: Will have a collaborative agreement with all attending Palliative Care physicians Workweek: Scheduling flexibility required to provide for an appropriate level of patient care coverage.
