Job Responsibilities:

The SBHC Nurse Practitioner provides primary and preventive health care, diagnosis and treatment of medical conditions and management of chronic conditions. Age appropriate reproductive health care. A HEADDSSS and an assessment of risk behaviors; Immunizations consent, provision and documentation. Population based assessments as well as responding to individual needs. On-site routine management of chronic conditions (asthma, diabetes, etc.), provided in consultation with the child's outside primary care provider or specialist as appropriate. On-site medications and prescriptions are provided for minor problems, acute problems; and chronic conditions. Laboratory evaluations are available as needed for both reproductive and general medical health needs.

Nurse Practitioner will assist Site Coordinators to ensure that regulatory and quality assurance protocols are operational for their SBHC and that day-to-day operations are as designated in the Principals and Guidelines for the NYSDOH. Nurse Practitioner will assist Site Coordinator in the regulatory review of charts, maintenance of communication logs with community physicians and referrals, maintenance of logs for laboratory and pharmacy, maintenance of regulatory documentation for equipment including refrigerators and on-site laboratory equipment (urinalysis, glucometer, pregnancy testing and hemoglobin testing) as well as all other nursing managed regulatory documentation. S/He will assist in the oversight of quality improvement quarterly assessments in coordination with medical director and staff. Nurse Practitioner will assist Site Coordinator to provide oversight to the operational needs of the SBHC including but not limited to ordering of office, medical, laboratory and pharmaceutical supplies.

Job Qualification

  • Master's Degree in a Nurse Practitioner Program, required.

  • Current license to practice as a Registered Professional Nurse AND a Nurse Practitioner New York State.

  • National Board Certification in specialty area, required. Certification to be obtained prior to next re-credentialing review.

  • Completion of a pharmacology component of not less than three semester hours or the equivalent AND has been instructed in New York State and federal laws relating to prescription and record keeping (3 hour course).

  • Has a collaborative practice agreement with a physician and designated protocols, both filed with the New York State Department of Education.

Previous experience relevant to the area of clinical practice, preferred.
