Insurance & Employee Benefits: * Health, Dental, Vision, 401(k) matching, PTO for full-time eligible employees * Weekly pay w/ direct deposit & Mobile shift access * Nurse and Employee of the month/Employee referral bonus * National company with over 300+ locations * HomeCare Pulse Employer of Choice/Joint Commission Accredited * Variety of in-home and assisted living assignments, procedures, and treatments/part of a team with RN Oversight Responsibilities: * Care for the individual based on personalized Plan of Care Informs the Director of Nursing and any other appropriate personnel of changes in the client’s conditions and needs * Counsels client and family through care process while updating family on a regular basis * Assures proper maintenance of clinical records in compliance with local, state, and federal laws * Participates in agency performance improvement activities and continuously staying informed about changes in the field of nursing and home health care Requirements: * Current unrestricted LPN/RN license in the state for which care is being provided * Minimum of two (2) year(s) of documented experience required * Reliable car/auto insurance/valid driver's license/BLS CPR * Chest X-ray or TB skin test (valid within one year from reading date) * Clear Nursing License and a clear Level 2 Background Check * Understands regulatory compliance with state and federal regulations * Adhere to HIPAA and maintain client confidentiality * Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull * Varied assistance is required depending on the mobility of the client.
