Reference #: 9241569Primary Function:The Lung Cancer Screening Program and Lung Nodule Program Nurse Navigator performs basic nursing skills, patient navigation, and data management largely in the outpatient setting, with a focus on enhancing both the patient experience and the quality of medical care provided by these Programs of Excellence. This Nurse Navigator will focus on navigating patients with lung nodules, lung cancer or who are at high risk for lung cancer, tobacco addiction, and related problems. A critical feature for success in this position is the ability to develop high-functioning interpersonal relationships with patients, physicians, scheduling services, for navigation of the Jefferson Health system.Assists with shared decision-making and education for patients undergoing lung cancer screening, supervised by a CRNP.Reviews test results and coordinates additional workup for patients, including follow-up scans, diagnostic procedures, and/or evaluation by additional subspecialists, Multidisciplinary clinics, or in the Sidney Kimmel Cancer Center.Documents test results and tracks patients who are due for follow-up in the Lung Cancer Screening Program or Nodule Clinic using state-of-the-art radiology tracking software.There are opportunities to participate in an active clinical-translational research environment and to gain experience in tobacco treatment counseling.Interacts with co-workers, visitors, and other staff consistent with the iSCORE values of Jefferson.Full-Time 40 Hours/Week DaysEducation: BSN required, MSN preferred.Licensure, Certification and Registration Requirements: Current PA RN License and current BLS certification required.Experience: Two (2) to three (3) years of clinical experience in a patient care setting required.Jefferson Health delivers state of the art healthcare services to patients throughout the Delaware Valley and southern New Jersey.
