The Float Clinical Nurse Coordinator will work in multiple programs (described below) to cover for vacations and sick times

Chronic Thromboembolic Pulmonary Hypertension ( CTEPH ) and Pulmonary Hypertension ( PH ) programs offer advanced and comprehensive evaluation and treatment approaches for patients with pulmonary hypertension, requiring medical or surgical intervention. As a key member of the CTEPH and PH teams, the responsibilities of the Clinical Coordinator include utilizing and applying current specialized knowledge and expertise to facilitate the effective management of clinical activities. The main responsibilities of the Clinical Coordinator will include triaging referrals for both: out of province and Ontario patients, ordering diagnostic tests (CTPA, BNP, ECHO, V/Q, PFTs, Carotid Doppler, Heart Cath, ECG, Pulmonary Angiogram and more), reviewing the results and discussing with a physician the best care plan, managing anticoagulation treatment, leasing with referring and family physicians, communicating with out of province referring team regarding the stages of investigation and/or status of patients and/or repatriation process, providing a comprehensive education to patients and their families, preparing patients for a surgery, collaborating with the medical, interventional, and surgical teams and continuously working on developing the new standards of care.

LungRAMP is an integrated Rapid Assessment and Management Program for patients with SUSPECTED lung cancer. The Clinical Coordinator will contribute to providing a seamless approach to care for patients in the LungRAMP program by being the main contact for patients in the system. The primary purpose is to expedite patient access to diagnostic services and resources, improve continuity and coordinate the timely, seamless and integrated care from the entry and assessment phases to the treatment phase of patient care throughout the cancer continuum. The successful applicant will work with physicians and other clinicians to coordinate the care plan, ensuring effective patient movement, to proactively identify issues to be addressed in preparation for surgery and discharge and to ensure that the patient is connected with medical and radiation oncology as required. They will also be a key support for MCC's by providing lists of new patients to the group for possible discussion, identifying complex and less usual cases for presentation and working with the multi-disciplinary team in preparing for MCC presentation.

RACE is a Rapid Assessment of Complex Pleural Effusions. The Clinical Coordinator in this program will be assisting and performing minor procedures (thoracentesis, tenckhoff catheter insertions, tenckhoff catheter removals); providing education and teaching on definitive management options (talc pleurodesis and tenckhoff catheters)- for both inpatients and outpatients; following-up with patients that have had newly inserted tenckhoff catheters within 48 hours to ensure homecare has been arranged; collaborating with LHIN representatives to ensure continuity of care for patients’ going from hospital to home; restocking the procedure room and clinic rooms with supplies

Primary responsibility for the Clinical Coordinators in the Toronto Centre for Liver Disease is the procedure room duties, which includes performing the venipuncture, IV insertion, albumin transfusions, paracentesis and thoracentesis procedures, wound care, recovering patients post procedures and liaising with clinicians regarding patient care. Other responsibilities include booking appointments in PHS, helping patient with the prescriptions, providing liver biopsy education

The Esophageal Motility Lab evaluates patients with dysphagia (difficulty swallowing), esophageal motor disorders, symptoms of gastroesophageal reflux (acid reflux), and difficult to diagnose esophageal problems. We also help adjust medications and therapies for patients with reflux disease as well as monitoring patients after their lung transplant surgery. As a member of the Motility team, the responsibilities of the CLINICAL COORDINATOR include: performing the esophageal motility tests, ambulatory pH studies and impedance studies, as well as utilizing and applying current specialized knowledge and expertise on recent advances in nursing practice to facilitate the effective management of clinical activities; performing teaching/educational responsibilities; participating actively in Quality Improvement initiatives and research projects.

QUALIFICATIONS

  • Bachelors of Science Degree in Nursing

  • Completion of a recognized masters degree nursing program preferred

  • Certificate of competency (licensure) from the College of Nurses of Ontario

  • Completion of Basic Cardiac Life Support (B.C.L.S.) program, Level II Rescuer certification

  • Up to three (3) years of relevant experience including experience working in an advanced Clinical practice role

  • Three (3) to Five (5) years Nursing experience in an Acute Care Medical/Surgical Inpatient Unit

  • Effective organizational, interpersonal and communication skills

  • Excellent decision-making and problem solving skills

  • Excellent time management skills

  • Excellent teaching skills with the ability to teach one-on-one and/or groups

  • Ability to function collaboratively within a multi-disciplinary team and/or independently

  • Ability to work in a self directed manner

  • Demonstrated awareness of and adherence to professional and ethical standards

  • Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand

  • Computer literacy

  • Research experience preferred

  • Membership with the RNAO preferred
