Atrius Health is seeking an experienced internal medicine/primary care Per Diem Registered Nurse to join our nursing team in our Kenmore practice. The schedule is Monday-Friday as needed.
The ideal applicant will provide practice management activities emphasizing a patient-centered, collaborative practice approach with primary care providers. Responsibilities include:
Utilize clinical nursing algorithms to participate in the management of chronic diseases such as diabetes, hypertension and cardiovascular disease.
Provide telephone triage to patients calling the practice for advice; determine appropriate disposition based on a comprehensive assessment of symptoms; provide home care recommendations as needed
Serve as a patient educator in areas related to self-care and chronic disease management, especially related to diabetes management
Has scheduled nursing appointments to provide patient education and individual nursing care within the scope of the professional nursing practice.
Triage walk-in patients
Support transitions of care and organizational initiatives to decrease hospital admission and readmission by providing post-discharge follow-up care
Provide wound care, injections, and assist with procedures
Education: Graduate of a state approved school of nursing. Current/Valid license to practice professional nursing in the Commonwealth of Massachusetts. BSN preferred. Must maintain Basic Life Support (BLS) certification.
Skills and Experience: Minimum of 2 years of experience working in Ambulatory Care, Medical Practice or medical-surgical inpatient experience. Direct Internal Medicine /Primary Care experience preferred. EMR and strong computer skills required. Excellent assessment and telephone communication skills required. Experience in Triage strongly preferred. Ability to work with all disciplines and in a busy environment.
