Bridge Home Health and Hospice Cardiovascular Nurse - Full Time in Walnut Creek, California
The Cardiovascular Nurse is a specially trained nurse responsible to oversee cardiovascular services and develop professional relationships with physicians, pharmacists, cardiovascular nurses and health care facility discharge planners.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Collaborates with home health team to identify potential patients for cardiovascular services.
Performs initial assessment to determine eligibility for cardiovascular services.
Coordinates patient care between pharmacist, clinicians, nurse practitioners and physician regarding initial drug therapy, any drug therapy problems, and changes in administration.
Keeps physicians, hospital discharge planners, and organization personnel, as appropriate, informed of home care patient progress following hospital discharge.
Responds to billing department needs and confirms that appropriate documentation is available, regarding cardiovascular services.
Performs direct patient care, in an efficient, effective, and clinically correct manner and as defined in the State Nurse Practice Act.
Conducts in-service programs regarding cardiovascular services and specific cardiovascular skills for nurses.
Maintains up to date knowledge regarding cardiovascular management.
a. Managing hypertension, congestive heart failure, chronic obstructive pulmonary disease, arrythmias and other heart conditions
b. Evaluating and monitoring heart devices, such as pacemakers and defibrillators
Maintains knowledge related to telemonitoring devices, EKG rhythms and management.
Serves as a resource for nurses and pharmacists regarding information about cardiovascular services.
Adheres to professional and ethical standards of nursing practice.
Performs other duties as assigned.

Registered nurse with current licensure to practice professional nursing in the state and valid CPR certification.
ACLS certification required.
Minimum of two (2) years’ experience in cardiovascular nursing.
Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and in good working order.
Professional certification of CCRN, CVRN, preferred
Graduate of an NLN accredited professional school of nursing. BSN preferred.
