Registered Nurse (RN) Home Health Case Coordinator:
The RN Case Coordinator completes individualized patient assessments, develops a care plan based on assessment/history findings, delivers patient-centered care that is culturally appropriate, evaluates a patient’s response to care, and revises as needed. The RN Case Coordinator delivers care as well as oversees and delegates responsibilities to other personnel (clinical & non-clinical).
RN Case Coordinator:
Manages patient flow to and from Home Health including admissions, resumptions, re-certifications, and resumptions to ensure timely patient care.
Develops the care plan that will direct nursing team in providing patient-centered care.
Functions as resource for RN/LPN team.
Collaborates daily with nursing team on needs of the patient and progress of patient
Proactively assesses the care team, including new staff’s stress level and ability to management patient assignments. Offers assistance as needed.
Coordinates training with volunteers and student assignments with appropriate unit staff.
Provides direct patient care following time-point as schedule requires.
Monitors patient care plans to ensure that they are updated appropriately
Follows up with physicians for needed orders and provides updates on patient progress.
Monitors patient’s chart for redundant, incomplete, or inaccurate information and resolves issues
Monitors patient care assigned to nursing staff to assure levels of care are maintained.
Completes daily departmental productivity/staffing/acuity reports.
Collaborates with team on patient progress. Identifies barriers to progress and reports on case load in case conference.
Collaborates with additional disciplines involved in patient’s care to aid in treatment
Promotes team building and acceptance of change for continual improvement of patient services.
Mentors and educates staff members as needed.
Participates in assigned meetings and committees.
Monitors team’s quality assurance activities, such as SHP, core measures, fall prevention, as well as other unit based priorities.
Participates in accreditation surveys and the maintenance of regulatory standards.
Facilitates problem solving, conflict resolution, and accesses the chain of command as needed.
Generally expected to function in this role when on duty.
Work on call as needed.
QUALIFICATIONS AND REQUIREMENTS
Education: Associate's Degree in Nursing (ADN) required.
Experience: Two years home health experience preferred.
License/Certification/Registration: Valid driver’s license and proof of automobile insurance required. Car for travel to and from home visits during shift required. OASIS certification required within 6 months of hire.
